Use this order form to replace older statues that may be damaged or broken. Use this order form to replace the rectangle base design with the circular base design. The original statue, or pieces of it, must be returned to the Chapter office. Verification by the chapter and payment must be received before production of this Replacement Statue will be processed.
The price is $300.00 and includes shipping and engraving.
